Access to Care Collaborative
Access to Care Collaborative (ACC) was established in 2001 to respond to the health care crisis and lack of access for basic health care by the uninsured working poor in the San Fernando Valley of Los Angeles County. Northridge Hospital Medical Center partnered with Providence Health System San Fernando Service Area, Mission Community Hospital, the Archdiocese of Los Angeles, MEND (Meet Each Need With Dignity) and Partners in Care Foundation, to develop a model to provide accessible, no cost primary medical care to uninsured, low-income.
The ACC works with adults through the largely untapped resource of volunteer retired physicians, nurses and other health care and lay professionals. NHMC assisted in recruiting volunteer physician-led teams of healthcare professionals to provide free medical care in the existing medical office space at the MEND free clinic located in Pacoima.
Today three clinics are held each month to address both the episodic and chronic healthcare needs of some uninsured working poor adults in the San Fernando Valley. ACC is providing earlier diagnosis of pre-diabetic or unmanaged diabetes as well as medical management to control this chronic condition. For more information call (818) 785-3489.
